Your Family Health Group

Ontario's family physicians are the foundation upon which our health care system is based. Your family physician is your first stop for primary care and your guide through this system. Your doctor's Family Health Group is a group of family physicians who are working together to give you and your family better access to quality primary care services.

Here's what it means to you

Quality and convenience:

  • A doctor or nurse will be available to you 24-hours a day, seven days a week.
  • Your family doctor will continue to see you during regular office hours.
  • As a registered patient, anytime another doctor in your Family Health Group treats you, your personal family doctor will be advised about the nature of your problem so that he or she will always be up-to-date about your medical condition.
  • As a registered patient, should you need assistance after-hours, simply telephone a registered nurse at your Family Health Group's Telephone Health Advisory Service.
  • The Telephone Health Advisory Service nurse will suggest ways to address your health
    concerns, such as recommending an appointment with your doctor, telling you how you can treat the problem yourself, or advising you to go to the emergency department.
  • You can rest assured that, with your permission, your doctor will receive a written summary of your call the next day, so that your doctor is kept fully informed about the nature of the problem and the advice you received.

It's easy to join a Family Health Group

Simply read and sign the enclosed Patient Enrolment and Consent to Release Personal Health Information Form and return it to your family doctor or use the stamped envelope to return the form by mail. By signing the form, you agree that, unless you're traveling or find yourself in an emergency situation, you will try to contact your Family Health Group doctor first when you have a health need. You also agree to allow the Ministry of Health and Long-Term Care to provide your doctor with information about health services you have received from family physicians outside your group. Should your doctor not be available, your Family Health Group will ensure that you will be directed to another appropriate health care practitioner. Your doctor may also choose to share part of your medical records with his or her group colleagues.
